The correct answer is option 1 i.e. She forgot lock.

1. The error lies in part 4 of the statement because of the wrong use of "Infinitive".
2. We need to replace "Lock" with "To lock" for making the statement grammatically correct.

Grammatical concept:- 

• The error is in ‘she forgot lock’. A preposition is required after ‘forgot’ to form a grammatically correct answer.
• The preposition 'to' will be used after 'she forgot' and it will become 'she forgot to lock' which is grammatically correct.

The correct sentence will be- 

She forgot to lock the door when she went out in a hurry.
